Summons to appear form and affidavit of service

Use this form to ask us to issue a summonsDefinitionA formal document issued by the tribunal which says someone must appear at VCAT on the date stated in the document. for a person to attend a hearingDefinitionThe time and place at which VCAT hears the parties argue their case and makes a decision. (usually by phone or video conference), and give evidence or produce certain documents. You will need to pay a fee for us to issue the summons.

You must swear an oathDefinitionA solemn promise made before God or a deity to tell the truth. It can be made using a religious text such as a Bible, Tanach or Koran, but does not require one. or make an affirmationDefinitionA solemn promise to tell the truth. in front of an authorised person that confirms the affidavitDefinitionA written statement where the contents are sworn or affirmed to be true.
